Apixaban 2.5mg Tablet x 1

Apixaban is an oral anticoagulant ("blood thinner") that inhibits factor Xa to treat and prevent blood clots, specifically reducing stroke risk in nonvalvular atrial fibrillation, and treating deep vein thrombosis (DVT) or pulmonary embolism (PE). 

It is taken twice daily, usually 2.5mg or 5mg, with or without food
